What is Lumbar Disc Hernia

Displacement of an intervertebral disc in the lumbar region, which can cause pain in the lower back and legs.

It is a localized displacement of material from an intervertebral disc that causes pain and numbness in the distribution of a nerve in its movement or sensation function.
In other words, they are fragments of disc out of their usual place that cause symptoms.

What symptoms can a lumbar disc herniation generate?

The most common symptom is lower back pain. Sciatica pain can be caused by a herniated disc and is the most frequent cause of this type of symptom.

The definition of hernia establishes that there may be pain located in the back or with radiation to the legs, numbness or loss of strength may also occur in the same paths. Occasionally it manifests itself with loss of urinary and/or fecal sphincter control, so the condition is considered a medical emergency. There is a minimal chance of erectile dysfunction.

Possible Causes of the Condition

Age-Related Degeneration

As people age, spinal discs lose hydration and elasticity, making them prone to wear and tear, leading to herniation.

Physical Strain and Injury

Improper lifting techniques, repetitive movements, or sudden trauma like falls or accidents can exert excessive pressure on lumbar discs.

Poor Posture and Sedentary Lifestyle

Prolonged sitting, and lack of physical activity weaken the muscles supporting the spine, increasing the risk of disc herniation.

Excessive Weight and Smoking

Obesity adds stress to the lower back, while smoking impairs disc healing and reduces spinal strength by limiting blood flow.

Genetic and Structural Factors

Family history, spinal deformities, or conditions like scoliosis can place extra stress on discs, contributing to herniation.

Frequently Asked Questions

Find answers to common questions about Lumbar Disc Hernia, including its causes, symptoms, treatment options, and recovery process. This section is designed to address your concerns and provide you with the clarity needed to make informed decisions about your health.

Each case is different and the therapeutic option must be individualized. In most cases, conservative treatment and lifestyle changes are recommended. Steroid injection in cases with specific characteristics has shown discrete and transient efficacy. Nonsteroidal analgesics show little or no benefit. There is no evidence to support that bed rest is beneficial for a favorable evolution of the disease.

There are different surgical treatments, the most common being microdiscectomy, open discectomy and tubular discectomy. The advantages of minimally invasive surgery lie in less blood loss during the procedure, reduced operating time, short hospital stay, and rapid recovery.

It is the million-dollar question. I will answer you with complete sincerity, there is no clear and precise consensus, however the guidelines recommend waiting a maximum of 6 weeks if there is no remission of pain. Performing surgery before 3 months gives better results. Likewise, the delay of 6 months or more in the onset of symptoms leads to poor results. The spine surgeon considers the patient's age, the duration and severity of symptoms, the location and morphology of the hernia, as well as the motor nerves affected, to be important.

Last but not least, without surgery 80% of patients will regain strength and 50% of them will have sensation again in the next 12 months. The severity of the weakness increases the chance of not recovering. The question that matters to us is: What is going to happen to your life in that time? It is usually a disease that affects people of productive age, causing significant limitations and disability.

The odds are completely inaccurate after 12 months. Surgery significantly improves pain and speeds recovery for daily activities. The usual recovery time with surgery is 4 weeks and without surgery is 12 weeks. So, after the operation you will be able to return to work in 1 week.

All procedures begin with removing the intervertebral disc fragments that cause compression of nerve structures. The treatment of choice at CICOVE is microdiscectomy (microsurgery) due to the extensive experience of more than 2 thousand patients successfully operated on. A 2.5 cm incision is made in your back and the area inside your body that is worked on is smaller than your little fingernail.

The surgery lasts about an hour and a half for each hernia intervened. The procedure can be performed on an outpatient basis and go home the same day (believe it or not), however most patients prefer to stay overnight in the hospital. On the same day you will be able to walk and go to the bathroom and you will be back at work in 7 days. These are the advantages of performing minimally invasive procedures.

Complications are inherent to any procedure of daily life. Think about what can go wrong while crossing a street: You can trip, you can get run over, you can be kidnapped, and you could even die trying. I would not finish listing all the catastrophic scenarios. However, almost the absolute majority of the time no incidents occur thanks to preventive measures. The same thing happens in surgery, a team checks numerous times that everything is in order and they take excessive precautions so that everything goes well.

The complications that are reported in the world literature that you should know are:

- Between 1 and 3% of patients may have exacerbation of symptoms (more pain, greater loss of sensation, etc.)
- 5% of people will not recover, regardless of whether they agree to perform surgery or reject it.
- Between 1 and 2% of those affected may have complications in the surgical wound such as: wound dehiscence, infection or seroma.
- 3% of patients may present with a dural tear.
- Only 1 in 2 thousand cases is death.

Incredibly, and despite the statistics, surgery is a safe procedure and most complications have a solution.
